Location: 117 Mechanic Street, Bellingham, MA On or about December 3, 2025, employees were operating the Toyota 8FGU25 forklift, serial number 13619, to transport countertop materials, such as and including granite slabs, where the industrial truck was not examined before being placed in service on a daily basis.
