29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. On or about July 15, 2025, during a residential roofing construction project located at 8222 South Bedford, Macedonia, OH 44056, the employer did not ensure that the ladder rails on the portable ladder used by employee(s) to gain access to the roof were extended 3 feet above the upper landing surface. A&I Roofing and Repair, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.1053(b(1), which was contained in OSHA inspection number 1807856, citation number 1, item number 1a and was affirmed as a final order on (3/19/2025), with respect to a workplace located at 17460 Clifton Blvd., Lakewood, OH.
