29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a) On July 7, 2025, and times prior, at Old Dominion Freight Line, Inc, 4530 15th Ave N, Fargo, North Dakota, in the loading dock area, employees loading, unloading and moving material to trailer bays operated forklifts with 6-foot fork extensions without new data plates showing the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. This could result in tip overs, exposing employees to crushed by or struck by hazards. b) On July 7, 2025, and times prior, at Old Dominion Freight Line, Inc, 4530 15th Ave N, Fargo, North Dakota, in the loading dock area, employees loading, unloading and moving material to trailer bays operated a Toyota forklift with MSI 8-foot forks without new data plates showing the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. This could result in tip overs, exposing employees to crushed by or struck by hazards. c) On July 7, 2025, and times prior, at Old Dominion Freight Line, Inc, 4530 15th Ave N, Fargo, North Dakota, in the loading dock area, employees loading, unloading and moving material to trailer bays operated Toyota forklifts with a Rice Lake weighing systems forklift scale without new data plates showing the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. This could result in tip overs, exposing employees to crushed by or struck by hazards.
