29 CFR 1904.33(a): The employer did not maintain the OSHA 300 log, the privacy case list, the annual summary and the OSHA 301 Incident Report forms for five (5) years following the end of the calendar year that those records covered. a) Harrison Bakery Inc., 1306 W Genesee Street, Syracuse, NY - Throughout the site. On or about June 18, 2025, the employer failed to provide copies of OSHA Forms 300 and 300-A, which were requested for calendar years 2022, 2023, 2024, and 2025 to date.
