29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: The employer does not protect each employee from fall hazards while ascending and descending potable extension ladders in that, the side rails of the ladder do not extend at least 3 feet above the landing surface. This most recently occurred on April 30, 2025, during the installation of roofing material at a job site located at 4481 West 227th Street, Fairview Park, OH; the employer did not ensure that portable extension ladders used on site extended at least 3 feet above the upper landing surface.
