29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend the required 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: 528 Bernardston Rd., Greenfield, MA: On or about April 25, 2025, the employees were using a multi position aluminum ladder configured as an A-frame ladder to access their roof top work location and the ladder did not extend 3 feet above the upper landing surface. Sadi Gonzales, then DBA as Pinguins Construction, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1583887, citation number 1, item number 4, and was affirmed as a final order on June 13, 2022, with respect to a workplace located at 260 Longmeadow St., Longmeadow, MA.
