Warehouse: On or about April 1, 2025, and at times prior thereto, a 10lb Amerex fire extinguisher had not been subject to their annual maintenance check(s). This condition potentially exposed employee(s) to extinguisher mechanical failure to put out and/or escape incipient fire hazard(s).
