29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): 81 Glenn Drive Tolland, CT. On or about March 31, 2025: a) An employee was not wearing fall protection and fell approximately 18 feet from the peak of a roof to the ground while documenting work in progress on a chimney. b) Shortly thereafter, a second employee was observed walking the peak of the roof to complete work on the chimney without fall protection. Safeside, Inc. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1712619, citation number 1, item number 3 and was affirmed as a final order on May 23, 2024, with respect to a workplace located at 288 Mile Hill Road, Tolland, CT 06084. Safeside Chimney and Duct Cleaning was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1511572, citation number 1, item number 2 and was affirmed as a final order on August 3, 2021, with respect to a workplace located at 203 Cedar Ridge Drive Glastonbury, CT 06033.
