29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 KN) per employee attached: Site: 133-25 37 Avenue, Flushing, NY 11354. a). On or about March 03, 2025, and at times prior thereto, BCG Glass Industry Inc., two employees engaged in metal panel installation on balcony dividers located approximately 10-100 feet above the ground below were connected to the same anchor point as part of their fall protection systems. The K-Strong UAF30090 and Werner A510000 anchors the employees were using were only rated for 5, 000 pounds per person attached, and exposed the employees to falling hazards.
