29 CFR 1904.29(b)(3): The employer did not enter each recordable injury or illness on the OSHA 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred. On or about February 27, 2025, an OSHA Form 301 or equivalent, for the following work-related injury was not filled out correctly for the calendar year 2025: a. Case number 1, had a work related injury or illness occurring on February 3, 2025 resulting in on job transfer or restriction days, was not entered on a OSHA 301 Incident Report or equivalent within seven (7) days of receiving information that a recordable injury or illness occurred. b. Case number 2, has a work related injury or illness occurring on February 14, 2025 resulting in a workplace fatality, was not entered on a OSHA 301 Incident Report or equivalent within seven (7) days of receiving information that a recordable injury or illness occurred.
