29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a) Processing room; On or about December 30, 2024, an employee's fingers were caught in the moving components of the Multivac Type R105 packaging machine (Serial No. 162229) resulting in an amputation, which was not reported to OSHA until January 3, 2025.
