29 CFR 1926.405(g)(2)(iv):Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: At a worksite located at 2115 SW Hook Farm Dr, Lees Summit, MO 64082, employees were exposed to electric burns, shocks, arc flash, and fire hazards. The employer exposed the employees to these hazards by allowing them to utilize an electrical extension cord with strain relief damage to power an electric circular saw. Urbina General Contracting LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.405(g)(2)(iv), which was contained in OSHA inspection number 1479599, citation number 1, item number 1 and was affirmed as a final order on 7-27-2020, with respect to a workplace located at 15900 W 96th St, Lenexa, KS 66227.
