29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired. a) On or about July 1, 2024, at the jobsite: employees were exposed to a 7 foot fall hazard while accessing and egressing the reclaim # 3 concrete tank 13' long 7' wide and 7' tall, inside the excavation on the east side, in that, the employer allowed employees to use a defective ladder which was missing its top rung. Sitek, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(15), which was contained in OSHA inspection number 1734071, citation number 1, item number 2 and was affirmed as a final order on 06-10-2024, with respect to a workplace located at North W Corner of Orange Blossom Trail and Sand Lake Rd, Orlando, FL 32809.
