29 CFR 1910.28(b)(3)(iv): 29 CFR 1910.28(b)(3)(iv): The employer did not provide that each employee be protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used. On or about April 23, 2024, in the Engine Room #2 at Americold Logistics LLC Samuel facility, the employee was exposed to a 13 ft fall hazard when the employer did not ensure that a self-closing door on a roof ladder operated to ensure the swing gate returns to the closed position after use.
