29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure. On or about February 5, 2024, and times prior thereto, extension cords, zip tied and wrapped around metal conduit, was used as permanent wiring to energize the machines, exposing employees to electrical shock hazards. a) The mistaway located on top of the Infinity machine was plugged into an extension cord wrapped and zip tied to conduit, b) At the Fadal machine an extension cord zipped tied and wrapped around conduit was used to energize a fan and computer.
