29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a) On or about January 19, 2024, the employer failed to ensure that each powered industrial truck operator was provided with training to operate a powered industrial truck safely in the maintenance department, exposing employees to struck-by and crushing hazards. Johns Disposal Services, Inc. was previously cited for a violation of this Occupational Safety and Health standard, which was contained in OSHA inspection number 1621763, citation number 1, item number 6, group number a, and was affirmed as a final order on March 8, 2023, with respect to a workplace located at 7311 Omega Circle, Franksville, WI 53126.
