29 CFR1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). Location: 208 Valley Road New Canaan, Connecticut a) On or about December 21, 2023, employees were exposed to cave-ins while working in an over 12 foot deep trench with vertical walls and no cave-in protection. b) On or about December 22, 2023, employees were exposed to cave-ins while working in an over 12 foot deep trench with vertical walls and no cave-in protection, which resulted in the fatality of the foreman.
