29 CFR 1910.1030(h)(1)(ii)(B): The medical record for an employee with occupational exposure did not include a copy of the employee's hepatitis B vaccination status, including the dates of all the hepatitis B vaccinations or any medical records relative to the employee's ability to receive vaccination as required by 29 CFR 1910.1030(f)(2): a) Northbrook Behavioral Health: The employer did not have any medical records or other documents for employees who did not consent to receive the vaccination due to prior receipt, that included the dates the vaccines were administered. Employees such as, but not limited to RNs, LPNs and MHAs are exposed to blood and other potentially infectious materials when administering sharps and when bitten and spat on by patients when doing restraints, on or about 11/7/2024. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
