29 CFR 1910.134(m)(2)(i): The employer shall establish a record of the qualitative and quantitative fit tests administered to an employee including the information required by 29 CFR 1910.134(m)(2)(i)(A)-(E). a. Foundations Health Solutions at 4660 Trueman Blvd, Hilliard, OH 43026: On or about August 9, 2023, the employer did not maintain fit testing records for employees who were required to wear respirators. The employer required employees to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus while providing care to suspected and confirmed positive COVID-19 residents.
