29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted. On or about June 7, 2023, at an establishment located at 302 South Bluemound Drive in Appleton, Wisconsin, subcontractors of Overhead Solutions, LLC were exposed to fall hazards of over 10 feet without utilizing any form of fall protection while engaged in commercial roofing activities. Overhead Solutions, LLC formerly known as Overhead Solutions Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1631608, citation 1, item number 1, group b, and was affirmed as a final order on December 28, 2022, with respect to a workplace located at 707 Elm Street, Neenah, WI 54956. Overhead Solutions, LLC formerly known as Overhead Solutions Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1590615, citation 1, item number 1, and was affirmed as a final order on September 15, 2022, with respect to a workplace located at 716 Foote Street, Seymour, WI 54165. Overhead Solutions, LLC formerly known as Overhead Solutions Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1583673, citation 1, item number 1, and was affirmed as a final order on September 15, 2022, with respect to a workplace located at 275 Peterlynn Drive, Wrightstown, WI 54180. Overhead Solutions, LLC formerly known as Overhead Solutions Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 959866, citation 2, item number 1, and was affirmed as a final order on April 29, 2015, with respect to a workplace located at 279 E. St. Joseph Ave., Green Bay, WI 54305.
