29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation: a) Jobsite located at or near 821-25 North 13th Street, Philadelphia PA- Exterior Large Scaffold- On or about May 22, 2023, the employer did not ensure scaffold supports were on base plates and mud sills, thereby exposing employees conducting masonry activities on the scaffold to fall hazards of approximately 26 to 29 feet from unstable scaffold. b) Jobsite located at or near 821-25 North 13th Street, Philadelphia PA- Exterior Northwest Scaffold- On or about June 9, 2023, the employer did not ensure scaffold supports were on base plates and mud sills, thereby exposing employees conducting masonry activities on the scaffold to fall hazards of approximately 6 feet from unstable scaffold. c) Jobsite located at or near 821-25 North 13th Street, Philadelphia PA- Exterior Northeast Scaffold- On or about June 9, 2023, the employer did not ensure scaffold supports were on base plates and mud sills, thereby exposing employees conducting masonry activities on the scaffold to fall hazards of approximately 6 feet from unstable scaffold. GTO Construction, INC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1511579, Citation Number 1, Item Number 1 and was affirmed as a final order on 7/29/2021, with respect to a workplace located at Messina Way and San Vencenzo Drive, Philadelphia, PA 19145.
