29 CFR 1910.134(k)(3): Training was not provided prior to requiring employees to use a respirator in the workplace: (a) Barge 141, Seward Dry Dock repair: On or about May 5, 2023 at at times prior to, employees required to wear respirators for confined space entry, were not provided initial and at least annual training thereafter, exposing employees to respiratory and physiological health hazards. The JAG Alaska Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(k) , which was contained in OSHA inspection number 1492766, citation number 1, item number 1(c) and was affirmed as a final order on March 15, 2021, with respect to a workplace located at 3306 Sorrell Rd Seward, AK.
