29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee: 89 Powder Mill Rd. Southwick, MA 01077: On or about March 8, 2023, an employee of Valley Solar LLC. was installing solar panels on a roof with a fall height of 9 feet 5 inches, and his personal fall arrest system (PFAS) was attached to an anchor point that was not capable of supporting 5,000 pounds. Valley Solar, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.502(d)(15), which was contained in OSHA inspection number 1546800, citation number 1, item 1a, and was affirmed as a final order on November 22, 2021, with respect to a workplace located at 17 Main St, Florence, MA 01062.
