29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator: On or about March 2, 2023, and at times prior thereto, the employer did not provide a fit test to all employees required to wear respirators. The employer required employees to wear N95 filtering facepiece respirators to protect against COVID-19 while working within the facility when confirmed positive COVID-19 residents are present, exposing employees to respiratory related hazards and Covid-19. Bell Nursing Operations, LLC was previously cited for a violation of this occupational safety and health standard (29 CFR 1910.134(f)(2)), which was contained in OSHA inspection number 1552259, citation number 1, item number 2 and was affirmed as a final order on 03-25-2022, with respect to a workplace located at 5000 Thayer Drive, Killeen, TX 76549.
