29 CFR 1910.29(b)(3): The employer must ensure guardrail systems are capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied in a downward or outward direction within 2 inches (5 cm) of the top edge, at any point along the top rail: On or about January 27, 2023, at an establishment located at 2099 Badgerland Drive in Green Bay, WI, in front of Receiving Pit A; the swing gate installed to provide fall protection at the Receiving Pit A was bent, broke through, and damaged to the point that the system would fail to prevent a fall into the exposed screw auger system below. Sanimax was previously cited for a violation of this Occupational Safety and Health Standard, which was contained in OSHA inspection number 1439428, citation number 1, item number 2b, and was affirmed as a final order on November 20, 2019, with respect to a workplace located at 2099 Badgerland Drive in Green Bay, Wisconsin, 54303.
