29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): On or about December 22, 2022, the employer failed to ensure that employees were protected by cave in of the trench by installing cave in protection, such as but not limited to trench boxes and shoring, at a residential job site located at: 3427 Wyoga Lake Rd., Stow OH 44224. NOTE: The employer was previously cited for a violation of this occupational safety and health standard or it's equivalent standard which was contained in OSHA Inspection #1364503, citation 1, item 1b, which was affirmed as final order on January 16, 2019, with respect to a job site located at 23250 Bryden Rd., Beachwood OH 44122.
