29 CFR 1910.119(e)(5): The employer did not establish a system to promptly address the PHA team's findings and recommendations Instance A: On or about December 12, 2022, the employer did not assure that the findings and recommendations made during the process hazards analysis regarding the hazards of toxic exposure to employees working in the control room of building #40 were resolved. Employees were exposed to inhalation/explosion hazards associated with the release of formaldehyde during different release scenarios in that the control room of building #40 was not maintained under positive pressure and had ventilation shut off to prevent the intrusion of formaldehyde into the control room during a release scenario.
