29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: On or about 10/19/2022, in the fabrication area of a facility located at 8052 Northwest 56th Street, Doral, Florida, employees were exposed to struck-by hazards when powered industrial truck operators were driving forklifts without receiving an evaluation of operators' performance within the last 3 years.
