29 CFR 1904.29(b)(2): The employer did not fill out or correctly fill out an OSHA Form 301 or equivalent for each recordable injury or illness: Amazon.com Services LLC - PHL7 at 560 Merrimac Avenue in Middletown, DE 19709: On or about October 27, 2022, the employer had not filled out OSHA Forms 301 in completeness for the calendar year 2022. Violative instances included but were not limited to: a) Case No. 113 (AFE - breathing difficulty/allergic reaction) occurring on 10/20/2022 had an OSHA Form 301 that was not filled out in a complete manner. Question 6 was not filled out and failed name the physician or other health care professional. Question 7 was not filled out and failed to list the location treatment was given away from the worksite. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
