29 CFR 1910.28(b)(11)(ii): (ii) Each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as follows: (see TABLE D-2) a) On or about November 9, 2022 in the Engineering Department at the infeed chain deck, the employer did not provide handrails for steps that had 4 treads and 5 risers, exposing employees to fall hazards. The Appalachian Wood Floors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.22(a)(1), which was contained in OSHA inspection number 1597921 citation number 1, item number 2, and became a final order on July 20, 2022, with respect to a workplace located at Appalachian Wood Floors, Inc., 838 Campbell Ave., Portsmouth, OH 45662
