29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) HBD/Thermoid, Inc. located at 1301 W. Sandusky Street, Bellefontaine, Ohio, 43311: On or about September 8, 2022, the employer or employer representative failed to contact the Toledo Area OSHA Office or the 24-hour OSHA number within 24-hours after an employee was hospitalized resulting from injuries sustained from a workplace injury/incident.
