29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: The employer is failing to accurately record illnesses on the OSHA Form 300 or equivalent for the calendar year 2022. At a worksite located at 4800 Country Club Drive, Jefferson City, MO 65109: Employees were exposed to toxic or hazardous substances: The employer did not enter correctly the following work-related injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2022. On July 21, 2022, U.S. DOL-OSHA confirmed that the employer had not placed a case on May 6, 2022, on the OSHA 300 or equivalent form. OSHA confirmed that an employee had been hospitalized after experiencing respiratory distress while at work. This employee later died as a result of exposure to a toxic or hazardous substance on May 9, 2022.
