29 CFR 1910.28(b)(6)(i): The employer did not ensure that each employee less than 4 feet (1.2 m) above dangerous equipment is protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard: a) Meadow Gold Dairy, Palletizer Belt: On or about June 14, 2022 and at times prior, employees were exposed to slips, trips, and falls while accessing and egressing steps to cross over the active and operational conveyor located between the palletizer and the case stacker rooms. DFA Dairy Brands Fluid, LLC (dba: Meadow Gold Dairy) was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.28(b)(6)(i), which was contained in OSHA inspection number 1528324, citation number 1, item number 2/C and was affirmed as a final order on 10/18/21, with respect to a workplace located at 312 3rd Ave. South, Great Falls, MT 59405.
