29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification that identified the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and which identifies the document as a certification of hazard assessment: On or about April 20, 2022, and at times prior thereto, the employer did not verify that a workplace hazard assessment had been performed through a written certification for the TIG welding operation inside Titanium pipes, requiring personal protective equipment (PPE) such as safety glasses, welding hood, steel toe shoes, long pants and sleeves.
