Text For Citation: 02 Item/Group: 002 Hazard:
29 CFR 1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a. On or about March 29, 2022, located in the Pallet Assembly Area, a maintenance employee was exposed to machine hazards associated with moving parts and pinch points while performing servicing and maintenance activities such as but not limited to cleaning, repairs, adjustments and unjamming on the Line 1 Viking Nailer, Model 505, Machine number 887M. The employer did not implement energy control application steps as the machine was not shut down and turned off to perform the work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)] and verification of isolation [(d)(6)], were not implemented to protect the employee from machine servicing hazards b. On or about April 12, 2022, located in the Pallet Assembly Area, employees were exposed to machine hazards associated with moving parts and pinch points while performing servicing and maintenance activities such as but not limited to cleaning, repairs, adjustments and unjamming on the Line 1 Viking Nailer, Model 505, Machine number 887M. The employer did not implement energy control application steps as the machine was not shut down and turned off to perform the work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)] and verification of isolation [(d)(6)], were not implemented to protect the employees from machine servicing hazards. Pallet Distributors Inc. dba ePallet, Inc. was previously cited for a violation of this Occupational Safety and Health standard, which was contained in OSHA inspection number 1558518, citation number 1, item number 2b and was affirmed as a final order on February 7, 2022, with respect to a workplace located at 10143 Copperhead Road NW, Sugarcreek, OH 44681.
