29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about April 4, 2022, employees were exposed to a trench cave in while performing fire hydrant replacement at 2770 Boston Road, Brunswick, Ohio 44212. The employees were working in a trench that was approximately 6 feet 5 inches deep and the employer did not ensure that they were using cave in protection.
