29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a. On or about March 24, 2022 at 200 West Railroad Street, Columbiana, OH 44408, the employer failed to ensure employees operating the Atlas Lathe located in the maintenance area were protected against coming into contact with the chuck in that no guard was in place. b. On or about March 24, 2022 at 200 West Railroad Street, Columbiana, OH 44408, the employer failed to ensure employees operating the Kalamazoo Horizontal Band Saw, model number 13AW, serial number M2931 were protected against contact with the blade in that no guard was in place. Columbiana Boiler Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1452654, citation number 01, item number 002 and was affirmed as a final order on February 11. 2020, with respect to a workplace located at 200 West Railroad Street, Columbiana, OH 44408.
