29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: On or about March 18, 2022, the employer failed to ensure that an employee who operated the Toyota Forklift was not evaluated every three years. XPO LOGISTICS FREIGHT, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1910.178(l)(4)(iii), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1349763, CITATION NUMBER 2, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON APRIL 9, 2019, WITH RESPECT TO A WORKPLACE LOCATED AT 2800 FORREST PARK ROAD SOUTHEAST, ATLANTA, GEORGIA 30354.
