Condition observed on or about 02/11/2022, at 20 Wilson Ave. Spring Valley, NY 10956: a-West Side of Roof: Employee #3 was engaged in roofing activities on a 6 on 12 pitched roof using pneumatic nail guns were exposed to eye loss/eye injuries due to struck by hazards as eye protection had not been utilized.
