29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6): (Instances 1) On or about November 13, 2022, an employee was exposed to machine hazards associated with the point of operation during servicing, clearing a jam and cleaning rubber from the Line 4 Ohio Knife (shear). The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the service work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. (Instance 2) On or about November 13, 2022, a management employee was exposed to machine hazards associated with the point of operation during servicing, the recovery of amputated fingers and cleaning of the Line 4 Ohio Knife (shear). The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the service work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards (Instance 3) On or about February 24, 2022, an employee was exposed to machine hazards associated with the nip points and being struck by the equipment while making alignment adjustments to the gripper machine located inside the Line 6 Festoon. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the service work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards Continental Tire The Americas, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(d), which was contained in OSHA inspection number 1223352, citation number 02, item number 001 and was affirmed as a final order on 9-20-2017, with respect to a workplace located at 11525 N Illinois Hwy 142 in Mount Vernon, Illinois.
