29 CFR 1910.502(r)(1)(i): The employer did not report to OSHA each work-related COVID-19 fatality within 8 hours of the employer learning about the fatality. Location - Domiciliary unit (DOM) at 200 Spring Rd., Bedford, MA, 01730 On or about September 27, 2021, an employee passed away due to COVID-19. The employer determined that the death was work-related on September 27, 2021. The employer notified OSHA of the fatality on September 29, 2021 at 3:30 PM which is more than eight hours from the date/time that they determined the death was work-related.
