29 CFR 1910.502(r)(1)(i):The employer did not report to OSHA each work-related COVID-19 fatality within 8 hours of the employer learning about the fatality: a) Sweeny Community Hospital, dba Sweeny Hospital District located at 305 North McKinney St., Sweeny, TX 77480. On or about September 21, 2021, an employee died of COVID-19. The employer did not report the work-related fatality to OSHA within 8 hours. The employer reported the death to OSHA on September 22, 2021.
