29 CFR 1910.502(q)(2)(ii)(A):The COVID-19 log did not contain, the employee's name, one form of contact information, occupation, location where the employee worked, the date of the employee's last day at the workplace, the date of the positive test for, or diagnosis of, COVID-19, and the date the employee first had one or more COVID-19 symptoms, if any were experienced. a) AMITA Alexian Brothers Medical Center located at 800 Biesterfield Rd. in Elk Grove Village, IL 60007: On or about August 26, 2021, the employer failed to ensure the COVID-19 log was completed with the necessary detail, including one form of contact information, occupation, location where the employee worked, the date of the employee's last day at the workplace and the date of the positive test for, or diagnosis of, COVID-19. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
