29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Joshua Rosas LLC Roof Ninja, on or about August 17, 2021, at the location of 9359 W. Sylvania Ave., Sylvania, OH, 43560: the employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing roofing operations from elevated surfaces greater than six feet from the level below. Joshua Rosas LLC Roof Ninja was previously cited for a violation of this occupational safety and health standard, 1926.501(a)(1), which was contained in OSHA inspection number 1472566., citation number 1, item number 1, and was affirmed as a final order on 07/20/2020, with respect to a workplace located at 16549 Milbrook Rd., Maumee, OH 43537
