29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: Location: Worksite Employees are not adequately trained to recognize and avoid hazards such as struck-by, caught-between, and improper use of personal protective equipment while working where the employer has not provided proper training in hazard recognition. The employer routinely fails to provide adequate training and instruction to each employee in the recognition and avoidance of hazards as demonstrated on or about August 13, 2021, on the 18 Crestway Road worksite in order to recognize and avoid hazards and exposure to injuries
