29 CFR 1910.502(q)(2)(ii)(A): The COVID-19 log did not contain, for each instance, the employee's name, one form of contact information, occupation, location where the employee worked, the date of the employee's last day at the workplace, the date of the positive test for, or diagnosis of, COVID-19, and the date the employee first had one or more COVID-19 symptoms, if any were experienced. a) Golden Hill Nursing & Rehab, LP dba Quality Life Services-New Castle located at 520 Friendship St. New Castle, PA 16101: On or about July 20th, 2021, in all areas of the facility, the employers 2021 COVID-19 log was missing the following elements: the employees name, the correct date of the employee's last day at the workplace, the correct date of the positive test for, or diagnosis of, COVID-19, and the correct date the employee first had one or more COVID-19 symptoms.
