29 CFR 1910.212(a)(2):Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: (a) On or about August 4, 2021, at the worksite located at 714 E. Keefe Ave., Milwaukee, WI, the employer did not ensure that guarding installed on a Webb lathe was adequately secured. The guarding that was installed on the Webb lathe was cracked and broken, thereby exposing workers to, at minimum, struck-by hazards.
