29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) Laser Craft, Inc. on or about June 16, 2021, at the address of 38900 Taylor Parkway North Ridgeville, OH: The employer failed to report the amputation injuries of an employee as a result of a workplace injury/incident. The employer or employer representative failed to contact the Toledo Area Office or the 24 hour OSHA number to notify the agency of an employee's amputation injuries within 24 hours of receiving the injuries. The employer later contacted the OSHA Office on June 23, 2021.
