29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit: On or about May 13, 2021, and at times prior, at the Kerr Dam, employees entered the inspection gallery without a completed entry permit to perform work and were exposed to hazardous conditions. Pursuant to 29 CFR 1903.19, within thirty (30) calendar days of the date of this citation, the employer must submit documentation describing the steps taken to ensure the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit to protect employees from permit-required confined space hazards.
