29 CFR 1926.1051(a): A stairway or ladder was not provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. a.) 4 Barnes Street, Ware, MA: On or about April 19, 2021, employees were observed engaged in roofing activities and were not provided with an approved ladder, ramp or runway to access the surface. They were provided with a shingle lift. Manuel Castro was previously cited for the same violation of this occupational safety and health standard 29 CFR 1926.1051 (a), which is contained in OSHA inspection number 1488278 citation number1 , item number 4 and was affirmed as a final order on 11/25/2020, with respect to a workplace located at 27 Cass Ave., West Springfield, MA.
